The classic varicose vein operation
The classic varicose vein surgery without stripping is the treatment of choice for extensive branch varicosis caused by large varicose veins, provided that the venous valves in the main trunk veins are still intact. The course of the varicose veins is marked with a color marker prior to the procedure and removed using a minimally invasive technique. Due to the very small wounds that do not require sutures, the method is also referred to as “miniphlebectomy.”
“Mini” in this context does not refer to the caliber of the vein, but rather to the minimization of invasiveness, particularly with regard to the incisions. The miniphlebectomy technique requires skin incisions of only 2–3 mm and is best suited for varicose veins for which foam sclerotherapy would be too elaborate and laser therapy would not be advisable due to the tortuous course of the veins.
In less extensive cases, the procedure can also be performed under local anesthesia. When larger areas of the thigh and lower leg are affected by varicose veins, short general anesthesia or twilight sedation is significantly more comfortable. Mobilization can begin 45 minutes after the surgery. Under normal conditions, discharge home is possible 90 minutes after the procedure.

Description
The surgical removal of large branch varicose veins has also continuously evolved over the years. While traditional methods used incisions up to one centimeter in length, stab incisions with modern techniques are only 2–3 mm long. Using a small hook, the vein located under the incision is hooked and pulled out. The vein is then cleared of surrounding tissue, severed, and extracted over longer or shorter distances simply by pulling.
If necessary, leaking connecting veins (perforator veins) between the superficial skin veins and the deep muscle veins can also be removed using the same method during the procedure. The small punctures in the skin are barely or no longer visible a few months after the surgery.
In contrast to foam sclerotherapy or treatment with liquid sclerosing agents, miniphlebectomy allows even large-scale findings with high-caliber varicose veins to be treated in a single session. Since the veins are completely removed, recurrence of these specific veins is no longer possible. The disadvantages and risks of sclerotherapy, such as pulmonary embolism or thrombosis, do not have to be accepted.
